During observation, ask the person doing the work to show the last awkward case they handled. Replace personal details before sharing it. Capture the starting evidence, the judgement made, the next recipient and what would happen if nobody acted. In the quote-request example, a missing supplier price may be less important than uncertainty over who can promise a delivery date. Mark that as a decision requiring an owner rather than immediately proposing another screen. This keeps discovery focused on the work that must become reliable, including work currently carried by memory or informal relationships.

Create one row for each meaningful action and one column for each role, then annotate the record boundary. An account manager may read an order only inside their assigned organization; that condition is part of the permission, not a note to remember later. Include actions that do not have an obvious button: search, download, export, background processing and granting another person's access. A model that covers editing but forgets export can still expose the same protected records.

Prepare checks for counts, key relationships and business meaning before writing the migration script. In the membership example, verify that a current entitlement points to the correct member and subscription, and that a cancelled membership is not accidentally reactivated. Check attachments and reference identifiers as well as the main table. Keep a list of records intentionally excluded, with the reason and the person who approved it. An unexplained difference is a failed check, even if the total number of imported rows looks reassuring.

Choose a troublesome order and walk it through the ledger: one item is withdrawn, the delivery address changes after approval, and the supplier replies late. Which work is already inside the estimate? Which assumption is now false? A useful review changes a specific line rather than reopening the whole proposal. Where evidence is missing, agree a small investigation and the decision it will unlock. Do not confuse contingency with permission to expand the product: contingency covers uncertainty within a promise, while a new promise needs a scope decision.

Have a receiving operator start from a clean environment and follow the documented steps. They should build the application, run the appropriate checks, apply migrations to a disposable database and restore a safe backup. Record where they needed an undocumented answer. Repeat only the steps affected by the correction, so the exercise remains focused. A recording of the original developer performing a deployment is helpful context, but it is not equivalent to the receiving team proving it can operate the product. The distinction matters most when the original team is unavailable during an incident.
